Referral Process

Eligibility for Admission to a Long-Term Care Facility

· 18 years of age or older.
· Canadian citizen or permanent resident.
· Valid Health Insurance.
· Be unable to have their needs met in their existing residence by community or other services.
· Needs that may be appropriately met by Kensington.

Process:

1. The person seeking admission ad/or representative must contact Toronto CCAC.
2. If a person is deemed eligible by Toronto Community Care Access Centre, the person and/or representative is asked to select three facilities of their choice.
3. During the selection process, the person and/or representative may wish to tour the facility. Call 416-963-9640 to arrange a tour.
4. When a suitable room becomes available, the person will receive a "bed offer" from the Toronto Community Care Access Centre.
5. If the person and/or representative acceps the bed, the Toronto Community Care Access Centre coordinates the details of admission.
